	Healing a wound can be like solving a puzzle and very challenging. &nbsp;It is up to me&nbsp;to figure out why a wound won&#39;t heal or why it is present in the first place.&nbsp; There are several&nbsp;reasons&nbsp;a wound won&#39;t heal, infection, poor circulation, uncontrolled diabetes,&nbsp;foot deformity, trauma and patient compliance.&nbsp;&nbsp; I love wound care because it calls into play all of&nbsp;my training&nbsp; in&nbsp;biomechanics, dermatology, infectious disease, primary care, medicine and&nbsp;radiology.&nbsp; I feel that I am using the full scope of my education.&nbsp; It is very rewarding to watch a wound evolve into &quot;normal&quot; skin.</p>
<p>
	In order to figure out what needs to be corrected to allow the wound to heal, I&nbsp;look at the wound from all angles.&nbsp;&nbsp;I evaluate the circulation to see if there is vascular compromise.&nbsp; If there is no flow,&nbsp;I refer to a vascular surgeon for intervention.&nbsp;&nbsp;I test for peripheral neuropathy and loss of protective sensation. I look the nutritional status of the patient to see if the diabetes is under control or if there are any protein deficiencies that would delay healing. &nbsp;I&nbsp;take x-rays&nbsp;and order MRI&nbsp;to evaluate for bone infection or abscess.&nbsp;&nbsp;I look at&nbsp;structural deformities&nbsp;such as bunions and hammertoes.&nbsp;&nbsp;Surgically correcting a&nbsp;hammertoe&nbsp;may relieve the pressure causing an&nbsp;ulcer. &nbsp;I look at&nbsp;shoe gear to see if it is appropriate and may prescribe diabetic shoes and multidensity inserts.&nbsp; Modifications such as a rocker bottom&nbsp; may prevent ulcerations.&nbsp; If there is a infection present&nbsp;I prescribe oral antibiotics or may have a patient admitted to the hospital&nbsp;for IV antibiotics.&nbsp;I&nbsp;consult with&nbsp;the infectious disease doctor&nbsp;to find the&nbsp;best antibiotic to treat the infection.&nbsp; &nbsp;I must be acutely aware when a diabetic foot&nbsp;infection&nbsp;requires immediate surgical debridement or amputation. &nbsp;It can make the difference between losing a toe&nbsp;or losing&nbsp;a leg.&nbsp; Sometimes a patient may be in denial about their wound or&nbsp;diabetes.&nbsp; Maybe a patient needs to make better eating choices, help monitoring their blood sugar or&nbsp;smoking&nbsp;cessation.&nbsp;&nbsp;I can help&nbsp;refer them back to their primary care doctor&nbsp;for management.&nbsp;&nbsp;Patients&nbsp;come in weekly for wound debridement.&nbsp; It is also a good opportunity to&nbsp;ask how they are doing will their blood sugar and offer encouragement. &nbsp;Chronic wounds may develop into a form of cancer and may need to be biopsied and surgically excised. If a wound has stalled I will use advanced therapy such as biologic grafts to faciliate the wound closure.&nbsp;&nbsp;&nbsp;&nbsp;After a wound has healed I provide diabetic foot care to prevent future problems.&nbsp;&nbsp;&nbsp; &nbsp;</p>

		Treat athlete&#39;s foot with an over the counter antifungal</li>

Applying ice to the area can make a vast diff erence, e.g., there can be a reduction in the inflammation and discomfort, as well as faster healing. Place a towel over the skin to protect it from frostbite. The ice can be left in cube form or crushed and put in a plastic bag. Many people have success using bags of frozen peas or corn since the contents conform to the contours of the heel. Styrofoam cups filled with frozen water can be peeled, and used to massage the affected area. If you experience continued heel pain, contact your podiatrist.</p>

<p>- If every American at risk for developing a diabetic foot ulcer visited a podiatrist once before complications set in, the US health-care system could save $3.5 billion in one year. Closing this gap in podiatric care would reduce health-care waste on preventable conditions, which reportedly starts at $25 billion, by 14 percent.</p>
<p>This estimation is a projection based on findings from a Thomson Reuters study published in the March/April 2011 issue of the Journal of the American Podiatric Medical Association (JAPMA). </p>
<p>The study's numbers were based upon the American population that has either commercial insurance (116 million) or Medicare (46 million) in the Thomson Reuters MarketScan Research Database. Sponsored by APMA and independently conducted by Thomson Reuters, the study measured the health-care records of nearly 500,000 patients with commercial insurance and/or Medicare. </p>
<p>&quot;The study's findings are astounding. If just one individual at risk for a foot ulcer sees a podiatrist once before a foot ulcer becomes apparent, they will have singlehandedly saved our country nearly $20,000 over three years,&quot; said Kathleen Stone, DPM, president of APMA. &quot;This data does not even include the 47 million uninsured Americans or the 58 million currently on Medicaid, who have a higher incidence of diabetes and complications. The bottom line is that seeing a podiatrist saves limbs and lives, and equates to billions of needed dollars saved for America's health-care system.&quot;</p>
<p>After comparing health and risk factors for those who had seen a podiatrist for care to those who did not, the commercial insurance group saved $19,686 per patient over a three-year period. The Medicare group saved $4,271 per patient over the same three years. Conservatively projected, these per-patient numbers support an estimated $10.5 billion in savings over three years ($3.5 billion a year). </p>
<p>Including today's podiatrist in the diabetes management team is a vital step to preventing ulcers and amputation. Recent Centers for Disease Control and Prevention statistics show that in 2006, more than 65,000 lower limb amputations were performed in the US due to diabetes-related complications. </p>
<p>Diabetes currently affects nearly 26 million people in the US, seven million of whom are undiagnosed. </p>

Th e stabilization provided by a cast also discourages muscle contraction. Plasterand fiberglass are the two most common materials used to make casts. Plaster may support the bone more specifi cally since it can be precisely molded to the patient. However, becauseof its weightiness, a plaster cast can prove cumbersome for the patient. In addition, the material must remain dry or its shape will become distorted. Fiberglass is sturdier and lighter in weight. It is also more breathable and longer-wearing than plaster. Both types of casts are applied over a few protective layers of cotton or synthetic material to protect the underlying skin.</p>
<p>HINT: Plaster for casts is white in color, but fiberglass comes in a selection of colors, designs and patterns. </p>

If you are prescribed a surgical shoe, put it on whenever you walk. Do not allow your foot to dangle, and try to keep your foot elevated to reduce pressure. If a walker or cane is recommended, don't be shy about using one. Ice the area for the fi rst few days post surgery. Doing so will help decrease swelling and bruising. Bandages should be kept clean and dry. If anything seems &quot;off &quot;about your recovery, be sure to alert your podiatrist. </p>

Each of the 26 bones of the foot experiences impact up to six times the force of gravity when performingaerobic exercises. Shoes for this activity should have an arch that compensates for the side-to-side motions and a substantial upper or strap that offers stability to the forefoot. Among a host of possible causes of this uncomfortable state are poor circulation, a side-eff ect of taking particular medications, stress, and smoking. These issues serve to constrict or lightly obstruct blood vessels in the feet and may cause the feet to change color. Wearing socks and staying indoors may not relieve the symptoms. If you are experiencing cold feet, avoid caffeine since it constricts blood vessels. If you smoke, quit.  Avoid wearing tight-fitting footwear.  Massage your feet to increase blood flow to the area. Should cold feet become an ongoing issue for you, be sure to see your podiatrist. </p>

Known as tinea pedis, it is caused by a fungus or a type of mold that flourishes in dark, warm, moist environments.  It can present as cracked and peeling skin between the toes and on the bottom of the foot, and it can appear as excessive dryness due to the flaking, scaling skin it causes.  If left unchecked, the fungus can go deeper into the skin and through cracks or breaks in the skin, allowing bacteria to enter and cause a bacterial infection.  </p>

<p>Hammertoe is a general term used to describe the buckling of any of the toe joints. Hammertoes may be rigid or flexible, subject to the joint's ability to move. With a flexible joint, you can straighten the toe with your fingers; a rigid hammertoe cannot be moved and may cause pain and distort the movement of the foot. The stress it causes to the ball of the foot may result in calluses. For mild symptoms, the use of a splint or pad to hold toes straight may be sufficient. If symptoms are severe, out-patient surgery may be necessary. After the procedure, a surgical shoe must be worn for several weeks. If you are having problems with hammertoes, see your podiatrist.</p>
<p><strong>Hint: The base of a hammertoe points upward and the end of toe points down.</strong></p>

What kind of problems might this cause?  Overpronation is one of them.  When a person overpronates,his or her foot rolls inward when taking steps. The arch then starts to flatten, which stresses other parts of the foot and can contribute to other foot problems, including heel spurs and tendonitis. Obesity, pregnancy,or a lot of pounding on feet can contribute to fl at feet. To see if you have fl at feet, take a look at your shoes at eye level and see if the sole is worn evenly. People with flat feet will wear one side of the sole out, closer to the heel, sooner than the other.  </p>
<p>Hint: People with flat feet should  wear shoes with a firm heel  counter. </p>

These muscles are important for walking. The Achilles tendon can become inflamed, most often as a result of overuse or arthritis, although inflammation can also be associated with trauma and infection. Tendonitis due to overuse is most common in younger individuals and athletes. Tendonitis from arthritis is found more frequently in the middle-aged and elderly populations. Symptoms usually include pain in the heel when walking or running. The tendon is usually painful to touch, and the skin over the tendon may be swollen and warm. Treatment usually involves ice, non steroidal anti-inflammatory drugs, rest and physical therapy.</p>
<p><strong>Hint: Maintaining strength and flexibility in the muscles of the calf will help reduce the risk of tendonitis.</strong></p>

I explain to patients that it is like try to stick a thin slice of deli meat on the bottom your foot and expecting it to stay put while you walk on it. The graft can take a few weeks to incorporate and may take one or more applications for the wound to heal.</p>

If the ligament was simply over-stretched, it is minor. If the ligament is completely torn, it can be debilitating. An injury to connector tissue that connects a muscle or group of muscles to a bone is considered a strain.  Like a sprain, it also may be minor or serious depending on whether it is overstretched or torn. The treatment for the two is essentially the same. The injured area should be immobilized, elevated and iced.  Anti-infl ammatories such as ibuprofen are recommended.  The podiatrist may want to use a traditional or air cast.  In some instances, surgery is necessary.</p>

This can result from shoes that allow the foot to slip forward in the shoe, causing the toes to hit the toebox. It is important to understand that a black toenail could also be due to the development of melanoma, a type of skin cancer, under the nail. Should you experience toenail blackness that does not improve, have it evaluated by your podiatrist.</p>

Trimming toenails to be tapered at the corners can predispose the nail to growinto the skin of the toe, with the nail curling down and digging into the skin. Initially, the area may become swollen, hard, and tender. Eventually, it can become infected. Nonsurgical treatment begun early on includes soaking the foot in warm water and wearing sandals. The edge of the ingrown toenail can be gently lifted, and cotton or waxed floss inserted between the nail and the skin. This should be changed daily until condition clears up. Surgical treatment may include having the nail partially or completely removed while taking oral antibiotics.<br /><strong>HINT:</strong> To prevent ingrown toenails, trim the nails no shorter than the edge of the toe. Ingrown toenails often occur from improper trimming, but poor foot structure, heredity, injury, and infection can be contributing factors. Puncture wounds are common in the foot, especially in warm weather when people go barefoot. Getting proper treatment within the first 24 hours is extremely important with puncture wounds because they carry the danger of embedding the piercing object under the skin. Research shows that complications of puncture wounds can be prevented if the patient seeks professional treatment right away.</p>

<p>There are many causes of flat feet, including decreased exercising and increased weight. Most people have a gap under the arch of their foot when they are standing. Those with flat feet either have no arch or a very low arch.Their feet may roll over to the inner side when they are standing, resulting in the feet pointing outwards. They may experience pain in their feet and leg joints. If flat feet cause pain, an evaluation with a podiatrist can determine the cause. The exact treatment will depend on the cause of the flat feet. Some common treatments are shoe inserts, rest, and anti-infl ammatory medication. Osteoarthritis is associated with wear and tear, brought on by age or being overweight. Cartilage covering the ends of bones becomes worn, resulting in inflammation, swelling, and pain. Rheumatoid arthritis, which causes the patient's immune system to destroy cartilage, can be triggered by infection or environmental factors. Post-traumatic arthritis can develop after an injury to the foot or ankle. It is similar to osteoarthritis, and can develop years after a fracture or severe sprain or dislocation. Following an injury, the body can secrete hormones that stimulate the death of cartilage cells. Tilting the foot to compensate for the pain can cause misalignment, which can worsen joint difficulties.<br />FAST FACT: Arthritis is the leading cause of disability in the United States.<br />While our specialty is feet, our work in podiatry can affect all parts of the body. Pigment-producing cells become cancerous and grow and divide at an uncontrolled rate. Ordinarily, melanoma usually begins as an abnormal mole. To distinguish a normal mole from a melanoma, an ABCD rule wasdeveloped.This guide characterizes the features of melanoma as Asymmetrical lesions, Border irregularity, Color variation, and a Diameter greater than the size of a pencil eraser. However, not all melanomas have features defined by this rule, so it is important to report any new growths or  changes in existing moles or lesions. When inspecting for melanoma, focus on the area between the toes, around and under the toenails, and on the soles of the feet. <br /></p>

There are numerous causes of this condition including injury from trauma and inflammation or nerve destruction by disease. Common diseases such as diabetes, vitamin deficiency, and alcoholism can also lead to neuropathy. Symptoms usually progress slowly over a period of time. Pain and numbness ensue, and the skin can become extremely sensititive. Although there is no proven treatment for neuropathy, there are some options available to help relieve the symptoms. If you suspect that you are experiencing neuropathy, make an appointment with your podiatrist.</p>

	<li>Select shoes that match the shape of your foot.  If your foot shape matches your shoe shape, you are on the right track to good shoe fit.</li>
	<li>When shoes contact the ground your feet elongate. Allow adequate space (3/4&quot; to 1/2&quot;) at the end of the shoe for your longest toe.</li>
	<li>Shop at stores that offer full service, including a good size and width selection and staff trained in proper shoe fitting and measuring.</li>
	<li>Shop for shoes later in the day; feet tend to swell during the day, and it's best to be fitted while they are in that state.</li>
	<li>Always try on both shoes, and walk around the store.</li>
	<li>Always buy for the larger foot; feet are seldom precisely the same size.</li>
	<li>Don't buy shoes that need a &quot;break-in&quot; period; shoes should be comfortable immediately.</li>
	<li>Try on shoes while you're wearing the same type of socks or stockings you expect to wear with the shoes.</li>
	<li>If you wear prescription orthotics -- biomechanical inserts prescribed by a podiatric physician -- you should take them along to shoe fittings.</li>
